Registration for New Students and for Students Transferring from Another DistrictTo open a new registration, the student's parent will need to come to
the Bacich or Kent Office
with the proper documentation. Please call the school's office by phone
before coming in, so that office staff can discuss the details with
you. Kent Middle School Registration is by appointment only, and
includes student placement testing for Math and Language Arts. The

When you come to the office, you will need to bring copies (for the office to keep) of the documents below:
- Student's birth certificate, passport, or baptismal certificate.
- Student's current immunization record, filled out and signed by the student's physician. Please see our Immunization Requirements page for more information.
- All documentation of your residency within the District boundaries. Please see the Proof of Residency page in this section for detailed information on the documentation that the District requires. Registration cannot begin until all required documentation is presented.
School Office staff will collect copies of the birth, immunization and residency documents for our files, and ask you for some basic information to open a
registration file:
- Student's Full Legal Name (Last, First, Middle), Gender and Date of Birth
- Name(s) of any siblings (Last, First) who are either currently
enrolled, were previously enrolled, or will be enrolling at the same
time
- Mother's Full Name (Last, First)
- Father's Full Name (Last, First)
- Home Address (must be in District boundary)
- Home Phone, with area code
- Guardian's Email Address (where registration login information will be sent)
- Entering Grade Level
Office staff will enter the above data into our
database system and create a registration record for your
student(s). After the data has been entered, we will print and/or email
you the login information so that you can complete all
registration pages (1--9). All registration pages must be completed before your student can be enrolled and scheduled for classes, so please finish as soon as possible. Thank you and welcome to the Kentfield School District.
